We designed and built the platform that helps NHS GP practices, Primary Care Networks and Integrated Care Boards assess their digital maturity, plan improvements, and track transformation over time. An HSJ award-winning product now supporting over 1,700 practices across England.
The Challenge
6,500 GP practices. No consistent way to measure digital progress.
The NHS has been pushing GP practices to adopt digital tools for years. Online consultations, NHS App integration, cloud telephony, electronic prescriptions, patient messaging. The list keeps growing and the expectations keep rising.
But practices vary enormously. Some are digitally advanced. Others are still working with paper-heavy processes and outdated workflows. For the organisations responsible for supporting them, there was no consistent, scalable way to understand where each practice sat on its digital journey, what needed to improve, and whether progress was actually being made.
Redmoor Health had deep expertise in NHS digital transformation and direct relationships with practices across the country. What they needed was a platform that could turn that expertise into a structured, repeatable, data-driven product.
What We Built
From assessment to action plan in 20 minutes
We built the Digital & Transformation Hub as a web platform hosted on AWS. It is the evolution of the Digital Journey Planner, an earlier tool we developed that Redmoor created in partnership with NHS England and which went on to win the HSJ Partnership Award for Primary Care Project of the Year.
The Hub starts with a structured 20-minute self-assessment covering 35 digital and transformation topics. Once complete, the practice unlocks a personalised Maturity Dashboard: a visual snapshot of where they are strong, where the gaps are, and what to do next.
From there, the platform generates tailored action plans with implementation guidance and links to curated resources. Progress is tracked over time, giving both the practice and the wider network a clear picture of how transformation is landing on the ground.
For Primary Care Networks and Integrated Care Boards, the Hub provides aggregated reporting and benchmarking across their practices. Digital leads can see exactly where their network stands and target support where it will have the most impact.

Under the Hood
Built for people who do not have time to spare
GP practices are under enormous pressure. Any tool that adds to the workload will be ignored. So the Hub had to be fast, intuitive, and immediately useful. The 20-minute assessment was designed to feel like a productive conversation rather than a compliance exercise.
We built the reporting layer to work at three levels: individual practice, PCN, and ICB. A Digital & Transformation Lead at a PCN can see which of their practices have completed assessments, where the common gaps are, and which areas need coordinated support. An ICB lead can see the same picture across an entire region.
The platform is hosted on AWS with multi-zone resilience and a 99.9% availability target. It contains no patient-identifiable data, which simplifies information governance and makes adoption faster. It is listed on the UK Government Digital Marketplace under the G-Cloud framework.
Impact
From a single tool to a national transformation platform
The platform began as the Digital Journey Planner, built in partnership with NHS England. It won the HSJ Partnership Award for Primary Care Project of the Year and was shortlisted for multiple additional HSJ Digital Awards.
The D&T Hub is now the evolved version: broader in scope, richer in features, and designed to support the growing ecosystem of Digital & Transformation Leads being funded through the ARRS scheme. Redmoor Health supports over 1,700 GP practices and works across multiple ICBs in England, and the Hub is the technology backbone that makes that possible at scale.
Across supported PCNs, the results are tangible. Income maximisation through stronger CAIP delivery and QOF performance. Better demand and capacity planning. Smoother patient access. Less wasted admin time. In a Lancashire pilot, targeted communications driven by data from the platform helped increase NHS Health Check completions by 156% across four GP practices, within existing appointment capacity.
